Pre-Arrangement Details

Before We Start... 

When you're ready to set an appointment and meet with us, where is the best place to do so?

Step One:
Vital Records/Death Certificate Questions 
For The Person That This Advance Plan is Being Made

These are the basic questions for information that funeral homes must obtain in order to file a death certificate in the State of Washington.  If a death occurs out of state, other questions may be required and will be asked/answered at the time, should the need arise.

Physical Address 
Depending on how long someone resides at a care center, the physical address used on the death certificate can still be the "family home" if circumstances warrant. We ask this question so that we're sure to follow-up during our meeting, ensuring that the death certificate is as accurate as possible.
A Social Security Number is also required to file a death certificate, but we obtain that either in-person or over the phone. It will be required for any advance-funding arrangements that are set up.
Please include city and state or, if born internationally, city and country. If the city of birth is unknown, the state or country will suffice.
If information is not known at this time, we can always fill it in later. Washington does allow us to also file death certificates with "Unknown" for this item.

Washington has specific requirements relating to funeral arrangements when someone is separated but not legally divorced. Letting us know that is the case helps us help you, especially for Advance Planning.
Please use maiden name / name before first marriage when applicable. For special circumstances (such as non-traditional couples) please discuss with your funeral director so that we complete this field accuratley.
Please do not use "Retired" -- The state classifies this question as "Type of work done most of working life".
Having a copy of the DD-214 / Discharge Documentation on file with the funeral home helps expedite our ability to ensure all VA benefits are obtained in a timely fashion.
If making arrangements for yourself, this should be the person who we will primarily work with to finalize remaining details with after your death. If you're completing this form for someone else, and you'll be the person we will be working with, this should be your information.

This can be as simple as "just a cremation" to "A full funeral at the Catholic Church". This helps us better prepare for our meeting with you to make sure we have all of the necessary information and resources available to answer your questions.
